Impact of Epidural Analgesia on labour: A Review

Pain relief in labour has always been surrounded with myths and controversies. Hence, providing effective and safe analgesia during labour has remained an ongoing challenge. Relief of labour pain has always been a point of debate in the society ever since the use of ether by J.Y. Simpson in 1846 to regional analgesia in 20th century. Impact of labour analgesia on the progress and mode of delivery has become a crucial subject of debate among obstetric and anesthesia care providers during the last few decades. A large number of unrandomized and retrospective studies have been done to assess the effects of epidural and parenteral analgesia on duration and mode of deliveries.
Epidural is a safe method to address the fear of pain associated with labour hence it should be explained to every primigravida during her antenatal period and she should be allowed to make knowledgeable choice.

Ovarian teratomas are one of the most common tumours accounting about 20% of adult ovarian tumours. They consist of either mature or immature tissue of all the three germ cell layers. Complications include – torsion (16%), rupture (1-4%), malignant transformation (1-2%), infection (1%), autoimmune hemolytic anaemia (<1%).Adhesion and perforation into bladder is one of the rare complications. We reported a case of 32year old parous lady with straining at micturition, narrow stream, passing hair i.e. pilimiction and turbid urine occasionally. On cystoscopy, hair ball visualized. CECT demonstrated an ovarian dermoid cyst invading the urinary bladder. Procedure--TAH with BSO and partial cystectomy done. The diagnosis of fistulous tract between ovarian dermoid with bladder was confirmed.

Primum atrial septal defect (ASD) is a common congenital cardiac anomaly that is typically repaired in the first years of life. Patients with isolated ostium primum atrial septal defects (ASDs) have benefited from important recent advances in the diagnosis, evaluation, and management of their conditions. Operative correction typically involves pericardial patch closure of the septal defect and closure of associated left atrioventricular valve cleft. This review will focus on thirty paediatric patients with sizeable ostium primum atrial septal defects who do not have other major associated cardiac defects.

A Rare Case of Herniation of Gravid Uterus

Incisional hernia is a frequent complication of abdominal wall closure and the management of pregnancy with a large incisional hernia with gravid uterus in its sac is challenging and requires a multidisciplinary approach. We report a case of 25yr old manual laborer, gravida 3 Para 1 live 1 abortion 1 who presented with an over distended abdomen and pendulous belly at 38 weeks of gestation. Examination revealed gravid uterus that herniated through the incisional hernia. Multiple erosions with crusting were seen over the abdomen. She subsequently had elective lower segment caesarean section and bilateral tubal ligation with repair of the hernia. She had uneventful post-operative recovery.

Pregnancy Outcome in Unicornuate Uterus with Rudimentary Horn

Background: Unicornuate uterus with rudimentary horn is one of the rare mullerian congenital anomalies (class-II) and consists of a relatively normal appearing uterus on one side with rudimentary horn on the other side. Pregnancy can occur either in unicornuate uterus or in rudimentary horn. If pregnancy occurs in unicornuate uterus, outcome may be spontaneous abortion, preterm labour or term delivery. Obstetric complications like breech presentation, IUGR, dysfunctional labour and caesarean section also are more common. If pregnancy occurs in rudimentary horn, rupture is a rule when un-diagnosed. Aim: To study pregnancy outcome in unicornuate uterus with rudimentary horn
Methods: This is a study of 8 cases diagnosed at laparotomy/ LSCS at Mamata general hospital over a period of 5years from November 2010 to october2015. Pregnancy outcome of 18 pregnancies in these 8 cases was studied retrospectively.
Results: 5 (27.8%) patients came with rudimentary horn pregnancy. 4(80%) were ruptured and one(20%) was unruptured. 13 (72.2%) pregnancies occurred in unicornuate uterus. 3 pregnancies (23.1%) aborted spontaneously. Preterm and term deliveries were seen in 3 (23.1%) and 7 (53.8%) pregnancies respectively. Obstetric complications present were breech-1(7.7%), IUGR-3(23.1%), oligohydramnios- 4(30.8%), intra-uterine death-2 (15.4%) and LSCS- 6(46.2%). There were 8 (61.5%) live births.
All the four ruptured rudimentary horn pregnancies presented around 20 weeks of gestation. Out of these one patient died of irreversible shock.
Conclusion: Pregnancy in unicornuate uterus with rudimentary horn is associated with poor pregnancy outcome. Especially pregnancy in rudimentary horn has considerable morbidity and occasional mortality.

Background: To study the management modalities and their outcome, morbidity and mortality in women with ectopic pregnancy.
Methods: 50 cases of ectopic pregnancy were diagnosed and recruited for the study after taking their consent for participation. Detailed clinical history and Serum β-hCG assay Transabdominal USG, Transvaginal USG were done.
Results: 35 (70%) patients were managed by laparotomy and 12 patients underwent laparoscopy. 17 patients had less than 1000 ml, 22 patients had hemoperitoeum of 1000-2000 ml and 4 patients had >2000 ml of hemoperitoneum intraoperatively. The most common site for ectopic was found in ampulla in 29(61.7%). 40 patients (85.1%) presented with ruptured ectopic pregnancy. 35 patients were managed by salpingectomy (74.4%), followed by salpingo-oophorectomy in 6 (12.7%) patients. 12 patients underwent laparoscopy.
Conclusion: Early diagnosis and prompt conservative surgical or medical management reduces maternal morbidity, mortality and also helps in preserving fertility.

Burden of Maternal Obesity on Congenital Anomalies: Implications and Future Trend

Obesity is linked to certain congenital anomalies viz. neural tube defects (NTD), congenital heart disease (CHD) and oro-facial anomalies. However the exact burden of obesity on congenital anomalies, its economic implications and future trend have not been well documented before. We present a thorough review of the current literature with deductive interpretations to arrive at the following observations. Congenital anomalies in general are a leading cause of infant and child mortality but they are on decline in many countries. However maternal obesity is on the rise. As a result, the share of maternal obesity contributing to congenital anomalies is likely to increase in future. Maternal obesity can therefore significantly contribute to perinatal mortality and its economic, social and psychological impact can be substantial.

Objective: To determine the frequency of various types of ventricular septal defects (VSD) and associated complications in local paediatric population.
Methods: A cross sectional descriptive study was conducted on children undergoing echocardiography in a single centre from May 2013 to January 2016 at CVTS Department Grant Medical College, Mumbai, Maharashtra. The data on all children below 15 years of age undergoing detailed transthoracic two-dimensional echo and doppler studies was reviewed. Cases with isolated ventricular septal defects were studied for age of presentation, gender, type, and associated complications.
Results: A total of 70 patients with congenital heart diseases underwent echocardiography and surgical procedure during this period. A total of 15 patients had isolated VSD (21.4%). Mean age was 3.1 ± 3.64 years (range: 4 years to 15 years). Females were 5 (33.3%) and males were 10 (66.6%). Of 15 patients, 11 (73.3%) were Perimembranous type, 2 (13.3%) were muscular type, 1(6.66%) were doubly committed subarterial type and 1 (6.66%) inlet VSD. Small, moderate and large VSDs were 5(33.3%), 6(40 %) and 4(26.6 %) respectively. Severe pulmonary hypertension was noted in 5 (33.3%) cases. Aortic valve prolapse was present in 5 (33.3%) cases and varying degrees of aortic valve regurgitation was seen in 3 (20 %) patients. Right ventricular outflow tract obstruction was found in 1 (6.66%) case. No Echo evidence of infective endocarditis.
Conclusion: Perimembranous ventricular septal defect was found to be the commonest type of ventricular septal defect. Large ventricular septal defects usually lead to severe pulmonary hypertension. Severe pulmonary hypertension was the commonest complication followed by Aortic Valve Prolapse and Aortic Regurgitation. Rest of the complications were rare.

Malignant Mixed Mullerian Tumor of Cervix: A case report

Malignant mixed Mullerian tumors of the cervix are very rare tumors, constituting only 0.005% of all cervical malignancies. It usually presents with abnormal vaginal bleeding in post-menopausal women. The tumor has high rates of metastasis, so careful management including surgery, chemotherapy and/or radiotherapy is done for these patients.
Here, we present a case of a 47 year old woman who present with an ectocervical polyp. The patient underwent hysterectomy and the polyp was diagnosed on histopathological examination as malignant mixed Mullerian tumor of the cervix.

Congenital Granular Cell Tumor in an Infant: a case report

Congenital granular cell tumor (CGCT) is a rare, benign soft tissue tumor seen in newborns.The tumor is quite uncommon, with an incidence of only 0.0006% and less than 250 cases have been reported in the literature so far. The lesion has a marked female preponderance, with the female:male ratio being 8-10:1. It usually occurs on the gingival of the anterior alveolar ridge, more frequently in the maxillary alveolus than in the mandibular alveolus.
In this case report, we present a congenital granular cell tumor, sent by a paediatric surgeon as histopathological tissue from the oral cavity of 1 day old male baby, delivered with a solitary polypoidal nodule projecting from the mouth and presenting as difficulty in breast feeding. The mass was sent in toto and was diagnosed on histopathological examination.

Schwannoma is a benign nerve sheath tumor composed of Schwann cells, which normally produce the insulating myelin sheath covering peripheral nerves. Here, we report a rare unsuspected case of 6 year old female child with schwannoma on the right side of the middle third of the dorsum of the tongue. This case highlights the clinicopathological features of schwannoma tongue and the importance of including schwannoma as one of the differential diagnosis among the lesions of tongue especially in children.

Adenocarcinoma of cervix accounts only for 10-15% of all cases of carcinoma cervix. Among adenocarcinoma, clear cell carcinoma is even rarer variant. It is very rare in children and intrauterine exposure to Diethyl Stilbesterol (DES) is the only known risk factor. Here we report a case of a 14 year old female with Primary Clear Cell Adenocarcinoma of Cervix without any history of intrauterine exposure of any drug/ DES.

Introduction: Morbidly adherent placenta is a life threatening obstetric emergency. There is an increase in the incidence of adherent placenta . An important risk factor is the placenta previa in the presence of a uterine scar Ultrasound is the primary tool for diagnosis but MRI is helpful in cases of inconclusive or if placenta percreta is suspected.
Methods: Review of case records (Retrospective case study) of women with adherent placenta during the years 2009 – 2015 at Amrita Institute Of Medical Sciences &Research Centre, Kochi.
To evaluate the high risk factors, management of morbidly adherent placenta and the maternal as well as fetal outcome in our institution.
Results: Out of 17 cases the incidence of placenta accreta increased from 0.15% in 2009 to 0 .26% in 2015. Placenta previa and LSCS were the major risk factors. Ultrasound and MRI was done in the vast majority.
The mean age of termination of pregnancy was 36.51 weeks. Elective surgery were done in 14 with Ceasarean hysterectomy in almost 50%. Haemostatic surgical measures were taken in all cases.
The complications included bladder injury, DIC and wound dehiscence. Blood and blood products were replaced .The average time of surgery was 2.21hours and blood loss was 2.34L.. There was no maternal or perinatal mortality.
Conclusion: In suspected cases, screening for placenta accrete should be done by 18- 24 weeks. Early admission, prophylactic corticosteroids, elective surgery around 36 -37 weeks in a tertiary care with multidisciplinary appproach helps to reduce the maternal and neonatal morbidity and mortality.

Introduction: Thyroid hormones play a critical role in fetal neurodevelopment in pregnancy and postnatal life. Hypothyroidism ( HT ) is common in pregnancy and is associated with several adverse and fetal outcome. The benefits of treating overt HT during pregnancy include improved obstetric and neonatal outcomes.
Methods: This cross-sectional study included 2492 pregnant women. The serum TSH were measured by ELISA and patient with 0.1-4.5 mIU/L were considered as normal , while patient with serum TSH of greater than 4.5mIU/L to 10mIU/L with normal fT4 were considered to be subclinical HT, patient with serum TSH >10mIU/l were categorized as overt HT.
Results: Out of total cases, 78.25% cases were found to have normal TSH while 15.56% cases were found to have SHT and 3.41% cases were affected with overt HT.
Discussion: Pregnancy involves several physiological changes that affect maternal thyroid function and thyroid hormone levels. HT in pregnancy is associated with increased risk of adverse pregnancy and fetal outcomes that can be ameliorated by adequate levo-thyroxine therapy. This study showed prevalence of subclinical hypothyroidism (SCH) to be 15%, while 3.4% antenatal women were found to have overt HT.
Conclusion: Timely management of HT and SCH is required to achieve a successful pregnancy outcome and serum TSH level is a simple and reliable indicator of thyroid status in pregnancy.

We present an unsuspected case of a young woman with post-coital bleeding who was suspected of carcinoma cervix clinically, but which on histopathology turned out to be a case of cervical TB.

Uterine Lipoleiomyoma: A five year clinicopathological study.

Background: Uterine fatty tumors are rare benign neoplasms. Amongst them, uterine lipoleiomyoma is considered as a rare variant of uterine leiomyoma, constituting less than 0.2% of benign uterine tumors. Our study was aimed to investigate the spectrum of clinical and pathological features of uterine lipoleiomyoma with emphasis on its presumptive histogenesis and the possible origin of this tumor.
Methods: This study was carried out in the Department of Pathology and Department of Obstretrics & Gynaecology, Vardhman Mahavir Medical College and Safdarjung Hospital, New Delhi, India for a period of 5 years (January 2011 - December 2015). We retrospectively analyzed 589 women, who had undergone surgery for uterine leiomyomas or any gynaecological malignancies. The data obtained consisted of patient’s age, clinical presentation, radiological features, histopathology and immunohistochemistry (IHC) findings. The data collected was analyzed by descriptive statistics.
Results: A total of 728 uterine leiomyoma and 10 lipoleiomyoma cases were seen. The patients age for lipoleiomyoma ranged from 30 to 75 years. Six cases were postmenopausal, three premenopausal and one reproductive age group woman. Two patients were diabetic, two had hypothyroidism while one had high triglyceride levels. On radiology, three cases were detected as uterine leiomyomas and in two cases a definite diagnosis of lipoleiomyoma could be established. Five patients had been operated for symptomatic leiomyomas (pelvic pain, menstrual disturbances) and rest five patients for gynaecological malignancies (cervical malignancy, endometrial carcinoma (2), ovarian teratoma, ovarian serous carcinoma). Its size ranged from 1 to 15 cm in diameter. Nine tumors were in the uterine corpus and one was in the cervix. No tumor displayed atypia, mitosis, necrosis, calcification, degenerative changes or prominent blood vessels. On IHC, the smooth muscle cells, pericytes, endothelial cells were positive for smooth muscle actin (SMA), desmin, vimentin while the adipocytes were positive for vimentin and were negative for estrogen receptor (ER), progesterone receptor (PR), Ki-67. There were no recurrences or tumor-related fatalities in follow up period of 6 months to 4 years.
Conclusion: Uterine lipoleiomyoma is a benign fatty tumor with favourable outcome and complex histogenesis.

Gastric cancer accounts for 8% of the total cancer and for the 10% of the deaths for all cancers 6. It is more common in males and has been associated with risk factors such as low socio-economic status, cigarette smoking, nitrites, chronic gastritis and H.pylori. In advanced stages, the patients of gastric cancer rarely metastasize to supraclavicular lymph nodes which is responsible for dismal outcome. FNA is imperative for a hasty diagnosis and if combined with immunocytochemistry can offer diagnosis with reasonable sensitivity and specificity.
To the best of our knowledge, a very few studies on FNA of the virchow’s node has been published that adequately highlights the diagnostic utility of integrating immunocytochemistry along with cytomorphologic findings.

BACKGROUND: To study the contributing factors of Acute Renal Failure on pregnancy in King George Hospital, A tertiary referral hospital, during 1 year period, January-2015 to December-2015
METHODS: Total cases of Acute Renal Failure complicating pregnancies in 1year (2015), were studied and analysed. Etiological factors, associated liver failure, coagulation failure, thrombocytopenia, sepsis, recovery status and feto-maternal outcome were studied and results tabulated.
OBSERVATION: The incidence of ARF in pregnancy is found to be 0.25%.Hypertension and its related complications were the most common. 48% of cases required either Peritoneal Dialysis or Hemodialysis and except for 2cases (5%) all had complete or at least partial recovery from failure.
CONCLUSION: ARF complicating pregnancies are not uncommon in tertiary care centres. If recognised and treated promptly recovery is >90% cases. Early identification and prompt management of preeclampsia and sepsis can prevent majority of ARF cases.

Background: AFLP is a rare, life-threatening pregnancy associated disorder manifesting in the late trimester or early postpartum period. Early diagnosis and treatment is very important due to multi organ involvement and high mortality.
Method: Ours is a retrospective case study of AFLP in Amrita Institute of Medical Sciences &Research Centre, Kochi during the 2years of september 2013 to august 2015.Clinical presentations and management details were extracted from records.
Results: Out of 6 cases, 5 were primi and 1 mutigravida. The mean gestational age of diagnosis was 36.67 weeks. The clinical features were vomiting in 2,jaundice in 5, pruritus in 2,encephalopathy in 2.Ultrasonography were done in all the 6 cases . Fatty liver in 3, ascites in 6 and bright liver were seen in 2. Labor induction with vaginal delivery in 1 and 5 had Caesarean delivery under general anaesthesia.
The post op complications were acute renal insufficiency in 5 , liver failure in 3, pancreatitis in 1, metabolic acidosis in 5 ,pleural effusion in 3, pulmonary edema in 5, pneumonia in 2, sepsis in 5,hypoglycemia in 3, DIC in 3 , ARDS in 3, MODS in 3, maternal mortality in 3 and perinatal mortality in 1. Ventilatory support were given in 5 and ionotropes were used in 3. Blood & blood products were given in 5. The average duration of stay in the hospital and ICU were 12.7days and 4.8 days respectively.
Conclusion: AFLP, a rare pregnancy related conditon with multi-organ failure and increased maternal and neonatal mortality. Aggressive management which includes maternal stabilization, rapid termination of pregnancy and intensive care support with multidisciplinary approach is important in reducing the complications associated with it.

An Unusual case of frontoparietal swelling in elder female: Diagnosed on fine needle aspiration cytology

A solitary skull metastasis of renal cell carcinoma as an initial presentation is a very rare event. The most common sites for metastasis are lung, bone, liver and adrenal glands. Fine needle aspiration is a simple non-invasive diagnostic tool for diagnosing skull lesions. Here we are reporting a case of 64 years old female presenting with skull swelling. Fine needle aspiration was done from the lesion and reported as metastasis of renal cell carcinoma on cytology . In this case report we are highlighting the importance of cytology in diagnosing metastasis from unknown primary tumor.

Patent ductus arteriosus (PDA) is one of the most common congenital heart defects. Failure of the ductus arteriosus (DA) to close within 72 hours after birth result in PDA. Clinical signs of ductal patency are murmur, tachycardia, bounding peripheral pulses, and congestive heart failure . Complications of a persistently patent DA after birth are heart failure, renal dysfunction, necrotizing enterocolitis (NEC) and altered postnatal nutrition and growth. Also risk of developing of chronic lung disease .Pharmacologic closure can be done by administration of intravenous indomethacin or ibuprofen .Other management strategies are cath intervention, and surgical ligation.

First episode of Seizure in a pregnant women is always presumed as Eclampsia. Diagnosis of neurocysticercosis in pregnancy is often confounded by fact that associated symptoms of headache, nausea & vomiting, visual disturbance, neurological changes and seizures maybe presentation of Eclampsia Syndrome. In this series we report pregnant patients with atypical features which were misdiagnosed as Eclampsia but they were later diagnosed to be Neurocysticercosis on Neuroimaging.

The outline of gastrointestinal diseases in India differs remarkably from the western parts of the world because of dietary habits of Indian people. On one hand, the developed countries are fighting with diseases such as obesity, osteoporosis, cancer, diabetes, allergies and dental problems. The Asian countries population are hitching with issues such as intestinal ulcers, skin diseases, vector infections and much more. This varies from region to region also. Some of the diseases which are common in India are rarely seen in western countries and vice-versa. As a result, there is a market for new therapeutic medicine, nutrients production and consumption of these products. The role of these supplements in the daily diet is essential or decisive is still not known clearly due to the implausible literature and current approaches in research fields. Molluscum contagiosum is an extremely contagious under-reported viral infection of the skin and occasionally of the mucous membranes which is caused by a double-stranded DNA poxvirus called the molluscum contagiosum virus. Molluscum contagiosum lesions are typically flesh-colored, dome-shaped, and pearly in appearance. Any area of the body can be infected by this virus, however most commonly involved parts are the face, trunk, arms, groin, and the legs. The areola and nipple are very rare locations to develop molluscum contagiosum. We herein describe an unusual case of molluscum contagiosum of the right areola in a 48-year-old woman and review the various clinicopathological characteristics of this ubiquitous condition.

Milky/ frothy serum samples can be received sporadically in cases of severe hypertriglyceridemia which can pose diagnostic challenge for the routine processing and result generation. We report difficulties encountered in processing of such sample in undiagnosed and comatosed 4 year old girl of new onset type 1 diabetes mellitus, severe diabetic ketoacidosis and hypertriglyceridemia induced acute pancreatitis. She had markedly elevated serum triglycerides [13,846 mg/dL] with high cholesterol [1267 mg/dl], serum amylase [150 U/L], serum lipase [230 U/L], pH 6.85 and bicarbonate 6 mEq/L. Arterial blood gas analysis and urine chemistry were performed for the initial diagnosis. After establishing the diagnosis she was managed with insulin therapy and conservative management.

Background : Dengue is the most important emerging tropical viral disease in the world today. The World Health Organisation[WHO] estimates 50 million dengue infections occur annually and almost half the world’s population lives in countries where dengue infection is endemic1 . Over the past several years, the epidemiological profile of dengue has been changing progressively and is currently characterized by an increase in the number of cases in children under 15 years of age2.
Aim and objective : To evaluate and co-relate the hematological and biochemical data in children during the evolution of dengue fever and to predict the severity of the disease.
Methods – It is a retrorespective study of 30 seropositive dengue cases below 18 years of age carried out in the period of Jan 2016 to Nov 2016. Their hematological and biochemical parameters were studied at Sri Balaji Action Medical Institute, New Delhi.
Conclusion – Awareness, early recognition and early diagnosis are important for favorable out come. Hematological and biochemical profile are very helpful tools for disease monitoring and can be helpful in prediction of prognosis. Thrombocytopenia is a feature of dengue.

Chikungunya fever is caused by Chikungunya virus (CHIKV)and spread by Aedes aegypti and Aedes albopictus.Clinical manifestations of chikungunya fever are variable and ranges from abrupt onset fever,skin rashes,minor haemorrhagic ,arthralgia/arthritis,vomiting,diarrhoea to severe manifestations like encephalitis,optic neuritis,seizures,retrobulbar neuritis and acute flaccid paralysis.Some clinical features are different in children and adults.This study was carried out to characterise the epidemiological,clinical,radiological and laboratory parametersalong with outcomes in all cases of chikungunya positive hospitalised children during 2016 epidemic of chikungunya in Delhi from august to november.

Study on Uterine Artery Doppler: Screening Tests in Hypertesive Disorders Of Pregnancy

OBJECTIVES: To Study and analyse uterine artery Doppler Resistance index as predictor of Hypertensive disorders in pregnancy at 18 to 22 weeks
METHODS A Longitudinal Prospective observational Study was done on 274 antenatal mothers attending the outpatient department of Obstetrics & Gynaecology, Rangaraya Medical College, Kakinada from January 2015 to October 2016 in 18 -35 yrs with inclusion criteria.Uterine artery Doppler resistance index (0.58 taken as cut-off) was recorded at 18 -22weeks of gestation. Appropriate statistical tests were performed using SPSS software version 17. Sensitivity, specificity, positive and negative predictive value of the tests was calculated were followed up till term for development of pre-eclampsia
RESULTS The incidence of hypertensive disorders detected in positive Doppler screening was16.2% (n= 44) where as 83.8 % (n = 230) of the study subjects were not hypertensive. Majority of the cases (67%) were found to have Mild preeclampsia (n =29) followed by severe preeclampsia in 17 % of cases (n = 7) and 8% of cases developed eclampsia and also 8% developed gestational hypertension (n = 4). 217 (79.2%) of the study participants were found to be having RI ≤ 0.58. Uterine artery Resistance Index as a predictor of hypertension has a sensitivity of 88.64% specificity of 92.17% positive predictive value of 68.42%, negative predictive value of 97.7% and diagnostic accuracy was 91.6%, this association is also found to be significant [p = 0.000 ( < 0.05)].
CONCLUSION The present study concluded uterine artery resistance index as a better test in prediction of hypertension in pregnancy to prevent or reduce maternal as well as perinatal morbidity and mortality.

Background: Gestational Diabetes Mellitus (GDM) is an emerging problem which affects a large number of pregnant women in India. Women with GDM have been shown to have abnormal lipid profiles with higher serum triacylglycerol concentrations but lower LDL levels. Early detection reduces adverse maternal and foetal outcome. The Lipid Accumulation Product (LAP) is an index of fat distribution and assessment which can be easily measured in an outpatient setup.
Methods: This study was a hospital based case control study. Cases were 30 pregnant women, newly diagnosed with GDM in their 2nd trimester while controls were 30 apparently healthy pregnant women without risk factors for GDM. Lipid accumulation product (LAP) was computed by multiplying a sex-specific estimate of waist circumference and the fasting triglyceride concentration, LAP = (WC [cm] - 58) x TGL [mmol/L].
Result: The mean age of patients with GDM was higher when compared to controls. (28.17 ± 3.34 vs 24.40 ± 3.07, p < 0.0001) The lipid profile showed a significantly higher value of serum triglycerides among cases while the differences in HDL were not statistically significant. Lipid accumulation product (LAP) in GDM patients was found to be significantly elevated when compared to controls. (133.43 ± 64.02 vs 62.89 ± 30.68, p < 0.0001)
Conclusion: Calculation of LAP can be done to identify the degree of risk for developing GDM. So LAP can possibly serve as in future as a screening tool for the diagnosis of GDM in an outpatient setting in resource poor settings.

Background: Breast cancer is the leading cause of death from cancer for females 40-44 years of age which is responsible for 19% of cancer related death in women.1 The exact percentage of NAC involvement in breast carcinoma cases depends on many parameters. Grossing methodology used can influence the pick up rate of carcinoma involvement in nipple areolar region in cases of carcinoma breast.
Methods: Aim of the study was to compare the pick up rate of involvement of nipple areola complex in carcinoma breast cases by extensive nipple areolar sampling vs routine methodology.
We studied 30 cases of breast carcinomas using extensive sampling methodology and compared the results with routine sectioning method. All the cases were having grossly uninvolved nipple areolar complex.
Chisquare test was used to know if there was an association between the grossing technique and positivity rate of nipple areolar complex. Chi-square table was prepared and statistics was calculated using SPSS software.
Result: There was a statistically significant association between the grossing methodology and pick up rate of nipple areolar involvement in carcinoma breast.
Conclusion: Our findings showed that, if Only gross involvement of nipple is taken into consideration large amount of occult NAC involvement will be missed out, which can be picked up only by extensive sampling of NAC. Increase in the accuracy of involvement of nipple areolar complex can further guide in the treatment of breast carcinoma cases by nipple sparing mastectomy.

Introduction
Cancer of the breast in women is a major health burden worldwide. It is the most common cause of cancer among women in both high-resource and low-resource settings, and is responsible for over one million of the estimated 10 million neoplasms diagnosed worldwide each year in both sexes. Limited number of studies have addressed the optimal surgical management of patients with breast carcinoma associated with benign or pathologic nipple discharge
Material and methods-
Thirteen breast carcinoma cases with nipple discharge were studied. Nipple discharge cytology was studied using giemsa method. Later all the mastectomy specimens were inked and sectioned from medial to lateral into no greater than 1 cm thick tissue sections and grossly examined. Extensive sampling of nipple areola was followed taking 12 sections from each nipple areolar complex.
Thirteen breast carcinoma cases without nipple discharge were chosen as control and also grossed and examined for nipple areolar involvement .
Results-
In our study, there were 13 cases with nipple discharge. Out of which, 10 cases were having benign cytologic findings and 3 cases showed malignant cytologic findings
Spss statistical software was used which showed no association between presence of nipple discharge and nipple areolar involvement also it showed no association between malignant nipple discharge cytologic findings and nipple areolar involvement.
Conclusions-
Nipple discharge itself has no association with nipple areolar involvement and should not deter a clinician from nipple sparing mastectomy if other factors are conducive. Careful patient selection can be done in cases with malignant nipple discharge cytologic findings, if negative margins could be achieved and appropriate adjuvant therapy given.
Further studies are needed to study the role of nipple discharge cytology in conservative mastectomy procedures.

Red Cell Parameters Variation in Third Trimester in Normal Pregnancy

Background
Haematological profile is considered one of the factors affecting pregnancy and its outcome. The study reported here investigated the variation in some haematological parameters during third trimester of normal pregnancy.
Subjects and methods
The study reported here is a cross-sectional study of 171 pregnant women in their third trimester who attended a tertiary health centre in India. Full blood count was performed using a Sysmex Hematology, a sixpart auto analyzer able to test all parameters.
Results
Overall, the values obtained were (mean ± standard deviation [SD]): Red cell mass, 4.222 ± 0.9329 cells/ml; hematocrit level, 34.865 ± 4.299%; hemoglobin concentration, 11.011 ± 1.6399 g/dL; Red cell distribution width, 15.801 ± 3.004%; white blood cells, 10.267 ± 3.102 cells/mm3; cell volume 84.261 ± 9.514 fL, corpuscular hemoglobin, 26.717 ± 3.566 pg; and corpuscular hemoglobin concentration, 31.524 ± 1.640 g/dL; neutrophil count, 68.391 ± 8.610 %; lymphocyte count, 22.080 ± 7.442%; monocyte count, 7.112 ± 4.957%; eosinophil count, 2.547 ± 2.636%. These values were compared with their non-pregnant control values and for correlation p-values were calculated. There was statistically significant association between all parameters except red cell mass and eosinophil count.
Conclusion
It can be concluded that pregnancy in women alters haematological indices such as haemoglobin, red cell mass, haematocrit, red cell distribution width, red cell indices, total leukocyte counts and differentials. The review highlights that pregnancy is a state characterized by many physiological haematological changes, which may appear to be pathological in the non-pregnant state.

Background: the study was conducted to assess the acceptance of PPIUCD in terms of age ,parity , education ,residence and any complications of PPIUCD in cesarean or vaginal delivery.
Method: this is retrospective study of 919 women who accepted PPIUCD insertion . analysis was made as per the data of their history available , and follow up records. those who didnt come for follow up were telephonically asked for any complaints due to PPIUCD.
Results: the acceptance of PPIUCD was higher in para 2 females in both cesarean and vaginal delivery( 38.3 %). it was more in age group 21-25(47.66%)and higher in educated group(56.9%). there was no case of perforation or any other major complication.
Conclusion:PPIUCD is a safe ,highly effective , long acting, cost effective method of contraception.

Serum Ca 15-3: A Useful Tumor Marker in the Prognostication of Locally Advanced Breast Cancer

Locally advanced breast cancer (LABC) continues to be a common presentation of breast cancer worldwide. Various serum markers have been studied to know the behaviour, metastasis and prognosis of breast cancer without going through any invasive procedure. CA 15-3 measures the protein product of the MUC 1 gene and is the most commonly used serum marker in breast cancer. The main use of serum CA 15-3 is monitoring therapy response and elevated levels are associated with metastatic disease and in non-metastatic patients, it is associated with poor prognosis. The aim of the study was to know the role of serum CA 15-3 level in diagnosis, prognosis and therapy response in patients with locally advanced breast cancer and to assess its relationship with other variables like tumor size, microscopic grade, nodal burden and Ki-67 antigen expression. Serum CA 15-3 levels significantly decreased after neo-adjuvant chemotherapy and mastectomy and it remained elevated in patients with suspected or hidden metastasis. Serum CA 15-3 levels exhibited positive statistically significant correlation with tumor size.

Patent Ductus Arteriosus ( PDA ) is one of the commonest acyanotic congenital heart disease.Surgical or device closure is done when medical management fails.Choice of management depends on size of PDA. A study was conducted to assess the type of surgical technique to be followed either direct ligation or division and suturing.10 Cases of PDA were studied from May 2012 to May 2017 ages ranged from 12 days to five years with youngest weighing 650 grams.There were no postoperative complications and death.In two patients division suturing done and in other eight ligation was done.Repeat 2 D echo after three months did not show recanalization in either group.Conclusion was both techniques are equally effective but division suturing was more difficult than ligation.

Background: To study or analyse the histomorphological spectrum and distribution of benign and malignant ovarian neoplasms in women of different age groups
Methods: 633 cases of ovarian tumors were studied over a period of 6 years(from July 2011 to July 2017) at the Department of Pathology, SDMH. All the cases were subjected to histopathological examination and IHC as & when required.
Results: In total,633 ovarian tumor specimens were examined. Out of which ,468 cases (73.9%)were benign, 23 (3.6%)were boderline and 142(22.4%)were malignant. Most of the benign tumors occurred between 31 & 40 years of age while malignant lesions presented commonly between 41 & 50 years of age. Most common histological types were serous cystadenoma (24.18%) followed by mature cystic teratoma (22.90%).The commonest benign tumor was serous cystadenoma & the commonest malignant tumor was serous cystadenocarcinoma. Serous tumors showed bilateral involvement more commonly than bilateral mucinous tumors.
Conclusion-: We noted that serous cystadenoma tumors are the commonest variety of ovarian tumors. Also the age of presentation of these malignant tumors was an earlier age as compared to others.

Lactational changes masquerading features of malignancy in breast: A Case Report

Introduction: Pregnancy associated breast cancer is defined as "The diagnosis of breast cancer is made during pregnancy or within one year afterward." The incidences of breast carcinoma are 1 in 3000 pregnancy in west. Risk is increasing because of increased number of late pregnancies. Young age, lactating breast (dense) and paucity of incidences not only delay but confuse the pathologist for accurate diagnosis.
Case report A 28 year lactating female with 2 months old baby, presented with breast lump. Lump was mobile, non-tender and of size 2x1 cm since 3 months and gradually increasing in size. On USG, it was benign. On FNAC, aspirate was milky mixed with blood, showing monotonous cells with abundant cytoplasm, bland nuclei in a secretary background. Final diagnosis was lactational changes in breast. After six months, lady came back with previous lump which was increased in size up to 6X5 cm. On FNAC, smear was having bizarre cells, reported as malignant and confirmed on histopathology. Patient was responded to chemotherapy and lump reduced in size.
Conclusion: Pregnancy associated breast cancer is on rise. High grade of suspicion and complete evaluation including trucut biopsy will lead to correct diagnosis.

A Placental Conundrum: Exaggerated Placental Site Tumor

Exaggerated placental site (EPS) is defined as a non-neoplastic condition in which there is proliferation of number of implantation site intermediate trophoblastic cells that infiltrates the endometrium and myometrium. This is a benign lesion and it’s an exaggeration of normal physiological process as the intermediate trophoblastic cells present in EPS are similar morphologically and immunophenotypically to those found in the normal implantation site. An exaggerated placental site may occur in association with normal pregnancy or an abortion. The significance of reporting this lesion lies in the fact that the cells of this lesion display an identical morphological features to the intermediate trophoblastic cells found in placental site trophoblastic tumour, placental site nodule & epithelioid trophoblastic tumour. To confirm the morphological analysis, we used immunohistochemistry panel comprising of Cytokeratin 18, p63 and ki-67(MIB-1). We here present a rare case of an exaggerated placental site in a 30-year-old female who was in her first trimester of pregnancy and presented with signs of incomplete abortion.

We present a rare case of unilateral duplication of the fallopian tube in a 36-year-old female during routine grossing of hysterectomy specimen. Microscopic examination revealed normal histology of bilateral fallopian tubes along with the accessory Fallopian tube on one side. Fallopian tube duplication may contribute to infertility, ectopic pregnancy and can be a cause for failure of sterilization

Background: 2D:4D ratio is a sexually dimorphic marker that act as potential marker of prenatal androgen exposure. Genital development in fetus depends on adequate and timely exposure to sex steroids. Assessment of fetal androgen levels and exposure are essential to predict normal development and future reproductive disorders. The 2D:4D ratio can be used to assess androgen action.
Aim: This study attempts to find out 2D:4D ratio of left and right hand in adult females and males and to find out whether any difference exists among these variables in males and females.
Methods: A total of 83 adults were included in the study. There were 51 females and 32 males. The distance of second finger and fourth finger was calculated from mid-point of proximal most digital crease to distal most end of the finger. The ratio was then obtained by dividing second finger length by fourth finger length.
Result: The mean 2D: 4D ratio of left hand and right hand in adult female were 0.98 and 0.99 respectively. The mean 2D: 4D ratio of left hand and right hand in adult male were 0.967 and 0.99 respectively. The males have lower 2D:4D ratio for left and right hand as compared to females. The mean 2D:4D ratio of left hand and right hand in left handed females were 0.99 and 1.01 respectively while mean 2D:4D ratio of left hand and right hand in right handed females were 0.987 and 0.99. The mean 2D:4D ratio of left hand and right hand in left handed males were 0.96 and 0.98 respectively while mean 2D:4D ratio of left hand and right hand in right handed males were 0.98 and 1.01.
Conclusion: Sexually dimorphic markers are variables that can be used as indirect marker of fetal androgen exposure. In this study the mean 2D:4D ratio in males for both left and right hand was lower than females in overall group and in case of both right and left handedness males and females. Male fetuses with higher testosterone levels will have a lower 2D:4D ratio. Digital ratio is an easy noninvasive parameter, abnormal 2D:4D ratio in females can predict excessive androgen action.

Choroidal Hemangioma: A Diagnostic Challenge

Choroidal hemangioma is an uncommon, benign vascular tumor and typically presents between second to fourth decade of life. It is usually asymptomatic but may presents with reduced visual function, metamorphopsia, and photopsia. We report a case of 33-year-old woman with sudden diminution of vision and floaters. On clinical and radiological findings, a suspicion of choroidal melanoma/metastasis was kept for which enucleation of eye was performed. But in contrast with the above findings a diagnosis of choroidal hemangioma was made on histopathology. Diagnosis of choroidal hemangioma is challenging as it can mimic serious and malignant intraocular lesions. Our case is interesting in view of its rarity and the diagnostic dilemma which it presents to the ophthalmologists.

Twin reversed arterial perfusion (TRAP) sequence with fetal intervention: Report of two cases

We report 2 cases of TRAP sequence diagnosed antenatally using three‐dimensional color Doppler ultrasound. Case 1 was a 44 year old G3A2 mother with monochorionic diamniotic (MCDA) twin pregnancy conceived after in vitro fertilization. Ultrasound imaging showed TRAP sequence at 18 weeks 6 days gestation with early onset intrauterine growth restriction of first twin and second twin being acardiac with flow reversal in umbilical artery and vein. Interstitial laser cord occlusion was performed at 20 weeks 6 days gestation. Serial ultrasound monitoring of growth of first twin done. The first delivered at 36 weeks 2 days gestation by cesarean section in view of poor growth. Baby weighed 1.55 kg at birth and had an uneventful neonatal period. Case 2 was 37 yr old primigravida with monochorionic diamniotic twin pregnancy conceived after in vitro fertilization. TRAP sequence diagnosed at 21 weeks 3 days gestation. Interstitial laser cord occlusion was performed at 25 weeks 5 days gestation and baby was delivered by cesarean section at 35 weeks 4 days gestation weighing 2.1 kg at birth. We conclude that early and accurate antenatal diagnosis is essential to improve outcomes in TRAP sequence. Minimally invasive intervention in the form of laser or radiofrequency coagulation of cord is safe and significantly improves the survival and long term neurodevelopmental outcome of pump twin.

Background: We examined changes in mean platelet volume (MPV) and platelet count according to the use or nonuse of therapeutic hypothermia for neonatal hypoxic ischemic encephalopathy (HIE).
Methods: We divided the newborn infants who were diagnosed as having HIE into the therapeutic hypothermia group (TH group, 24 infants) and non-therapeutic hypothermia group (non-TH group, 31 infants) and examined perinatal information and platelet count and MPV on admission, postnatal day 5, and postnatal day 7.
Result: In the TH group, an increase in MPV was found from admission to postnatal day 5 (p=0 .03), whereas no change was found from postnatal day 5 to day 7 (p=0 .36). In the non-TH group, MPV increased from admission to postnatal day 5 (p<0 .01) and did not change from postnatal day 5 to day 7 (p=0.71). Within the two groups, a high level of MPV was found only on postnatal day 7 in the TH group (p<0.01). A negative correlation was found between platelet count and MPV on admission and on postnatal days 5 and 7 in the TH group (admission: p=0.02, R=0.46; postnatal day 5: p=0.03, R=0.43; postnatal day 7: p=0.03, R=0.42), but no correlation was found at any time point in the non-TH group (admission: p=0.93, R=0.11; postnatal day 5: p=0.07, R=0.28; postnatal day 7: p=0.58, R=0.13).
Conclusion: Even if therapeutic hypothermia was implemented early after birth, we presumed that the bone marrow maintained the platelet count by increasing platelet production as the platelet count decreased.

Torsion of ovarian cysts and tumour are fairly common and widely known but torsion of the normal ovary in left-side is extremely rare (1). A very high degree of clinical suspicion is required for its prompt diagnosis and management. We are reporting a case of left-sided ovarian torsion in a 2½-year old child who presented with pain abdomen. She was successfully managed with left-sided oophorectomy. During a wide review of literature, we failed to find torsion in a normal ovary in a 2½-year old child.

Predictive Clinical Score for Early Diagnosis of Late Onset Sepsis

Background: Neonatal infections account for 40% of all neonatal deaths in developing countries. There is paucity of data regarding the scoring purely based on clinical signs for the prediction of sepsis and hence the intention of the study is to develop a scoring system for risk predictability and early management
Methods: All neonates developing signs of sepsis beyond 3rd day of life, underwent septic screening, with 2 or more screening parameters positive, blood cultures and csf were drawn and analysed. We calculated sensitivity, specificity, positive predictive value, negative predictive value and likelihood ratio of individual sign and constellation of signs. A score was formulated based on these parameters.
Result: Study cohort included 159 neonates with 342 symptomatic events. Respiratory distress, fever, abdominal distension and refusal of feeds were the most common presenting complaints. Abdominal distension had the highest positive predictive value and likelihood ratio and highest score. distention along with Increased abdominal girth and altered gastric aspirate together achieved highest score. E.coli was the most common organism isolated.
Conclusion: The study shows that when ready access to laboratory tests is meagre, the score based on clinical signs can significantly contribute to the diagnosis of late-onset septicemia and its early management and hence improve neonatal survival.

Ultrasound Investigation of Scar Endometrioma in a Patient with Bladder Exstrophy

Extra pelvic forms of endometriosis is common, however, endometriosis after bladder exstrophy surgery has rare been reported, and here we present one of the first reported case. A 32-year-old woman with a history of bladder exstrophy was aware of a painful induration at the operative scar located in the mid lower abdominal wall, and presented at an Australian hospital. She had had endometriosis in 2011 and she was managed with medical treatment. When she was born underwent surgery in Recife, Pernambuco – Brazil. She was followed-up for more than 25 under the care of a pediatric surgeon. She had several urinary infection and partial incontinence. Otherwise she was a normal woman with boyfriend. She went to college and graduate in architecture. She moved to Australia two years ago and is being follow-up by urologist. Her main concern is her partial urinary incontinence and the willing of being pregnant. In the past two weeks she presented with increasingly colic abdominal pain, which seemed to be located under the lower abdominal scar. Her abdominal ultrasound investigation revealed “There is a solid lesion demonstrating mild internal vascularity in the lower anterior abdominal wall, deep to the surgical scar and 6 cm inferior to the umbilicus. This could represent an endometrioma”. She has been managed with medical approach. The desire of sexual intercourse and pregnancy is being discussed. Artificial insemination is other way of fulfilling motherhood dream.

Sertoli-Leydig cell tumours (SLCTs) are rare sex cord stromal neoplasms of the ovary. They are more commonly seen in the reproductive age group and are rare in the postmenopausal period. The average age of presentation is 30yrs. We report a rare case of Leydig cell tumour with an interesting presentation in a postmenopausal woman.
A 60-year-old woman, postmenopausal for 12 years, presented with hirsutism, male pattern baldness and deepening voice over the past five years. On general examination, she had hirsutism, male pattern baldness with hoarseness of voice. Her blood counts and metabolic profile was normal. However, endocrinology work-up showed increased levels of testosterone (980ng/dl) (N 15-70 ng/ml). MRI Abdomen and Pelvis revealed a left adnexal mass measuring 4.1x4.0x3.3 cm which was heterogeneous with a centrally hyper intense area. No invasion in the surrounding structures was seen. No ascites was present. Total Abdominal Hysterectomy with Bilateral Salpingo-oophorectomy was done. Histological examination and immunohistochemical findings were of Sertoli-Leydig cell tumour. Postoperatively the symptoms of the patient abated and the levels of testosterone came back to normal.
